We only have death records for person's who died in Tippecanoe County, Indiana.
Please provide the following supporting documents with your application:-Mother of Deceased: Your ID-Father of the Deceased: Your ID-Grandchild of Deceased: Your ID and Documentation of Relationship-Sibling:- Your ID and Your Birth Certificate-Spouse: Your ID and Your Marriage Certificate-Child of Person Named on Record: Your ID and Your Birth Certificate
--Attorney's Office or Government Office: Your ID and letterhead for your department requesting certificate.
-Other: Your ID and your paperwork.
Note: There will be a $12.00 replacement fee for a copy of a certificate if the original is misplaced or lost. A death certificate is sent priority mail.
Once this application is received with proper Identification, your order will be processed. Domestic Orders take about 7-10 business days for you to receive the certificates in the mail.
A Certified Copy has the most information and is a legal copy. This document includes a seal.
