Immunizations

Immunizations are available by appointment only. 

Appointments may be scheduled on Tuesdays, Wednesdays, and Thursdays from 9:00 am - 11:45 am and 1:30 pm - 3:30 pm.

Due to the cost of printing of immunization records we charge 25 cents per record.

  • 2 Hepatitis A
  • 2 MMR ( Measles, Mumps and Rubella)
  • 2 Varicella (Chicken Pox)
  • 3 Hep B (Hepatitis B)
  • 4 Polio
  • 5 DTap

Grades 6-11

  • Meningitis #1
  • Tdap
  • Any missed Kindergarten Doses

Grade 12

  • Meningitis #2

Recommended

  • Flu
  • HPV (male and female)

Recommended Schedule for Childhood Immunizations

  • 2 Months: DTaP #1, Hib #1, Polio #1, Hepatitis B #1,Prevnar #1, Rotavirus #1
  • 4 Months: DTaP #2, Hib #2, Polio #2, Prevnar #2, Rotavirus #2, Hepatitis B#2
  • 6 Months: DTaP #3, Hib #3, Hepatitis B #3, Prevnar #3, Polio #3
  • 12 Months: MMR #1, Varicella#1,Prevnar #4, Hep A#1
  • 15 Months: DTaP #4, Hib #4, Lead Screening
  • 18 Months: Hep A #2
  • 5 Years: DTaP #5, Polio #4, MMR #2, Varicella #2
  • 11-18 Years: Tdap, Meningitis, Gardasil (male and female)
