| Birth and death certificates are issued on Monday through Friday 8:00 a.m. to 4:15 p.m. except on county holidays. There is a $8.00 fee for a certified copy of a birth certificate and $10.00 fee for a certified wallet-size copy of a birth certificate. There is also a $10.00 fee for a death certificate. Warning: False application, altering, mutilating or counterfeiting Indiana Birth or Death Certificates is a criminal offense under Indiana Code 16-37-1-12. |

ID REQUIRED: You must fax or mail a copy of your ID such as driver's license or state ID and your tracking number which you will recieve after you have completed the application process.

2. Mail in an Application...
- a self-addressed stamped envelope
- a copy of your ID such as driver's license or State ID
- a money order. No personal checks accepted.

To receive a copy of a certified birth certificate you must have your ID such as driver's license or State ID. We can accept cash, money orders, MasterCard or Visa only. No personal checks accepted. Please allow at least 10 to 15 minutes to pick up a certified birth certificate.

The Vital Records Division has birth and death records dating back to 1882. Records are kept in our office only for persons who's birth or death occurred in Tippecanoe County. You may contact the Indiana State Department of Health's Vital Records Division for birth/death records that have occured in the state of Indiana. |
