HOW TO SEARCH THE DATABASE
To search the list of available programs in the database, click the link on the left hand side of the menu system titled "Search Programs". The search screen provides you with 3 options for displaying programs from the database, by the type of program, by a specific church or organization name, or by the side of town the program is taking place.

To begin your search, click the option button next to the search type and select an item from the dropdown list of available search parameters, then hit the search button below.  
         

A message will be displayed advising you of the number of programs in the database that meet your search criteria;



A brief description of each program including the program name, location, and availability will be provided to you.  If you are interested in obtaining more detailed information regarding a program in your search, click the name of the program which will link you to a detailed information page with program and provider specifics outlined.

 

Once on the detailed information page for a program, you may return to your search results page to view other programs simply by clicking the links provided titled "Back to Search Results".


If the search parameters you select yield no results from the database, a message will appear indicating no programs are currently available;



If you are searching by church or organization,  the detailed information of the group will be displayed so that you may contact them directly to inquire about the status of programs they provide.  Due to the large number of programs provided by some churches and organizations, they may choose not to list their programs individually in the database.



HOW TO ADD YOUR CHURCH OR ORGANIZATION TO THE DATABASE
To add your church or organization to the database, click the link on the left hand side of the menu system titled "Churches & Organizations".  A series of textboxes and drop down menus are provided to allow you to enter detailed information about your church or organization.

If a red asterisk appears next to a field name on the screen, this denotes a required field. 



If you attempt to save the record and leave a required field blank, an error message will appear at the bottom of the screen telling you that this information must be completed in order to proceed.  

 

Once you have completed all the information and filled in all of the required fields on the screen, hit the submit button at the bottom of the screen.  Upon successful submission of the information, you will automatically be redirected to the program information page which will allow you to begin entering information on the programs your church or organization currently provides.

To verify your information has been saved correctly,  your church or organization name will appear in both the drop down lists on the program information page and search page. 



Note: You will only need to enter your church or organizations details into the database ONE time. 

Currently the system does not allow you to correct data entered in error or make updates to the information you initially provided.  In the event that an error has been made or there is a need to provide updated information on your church or organization, please contact either Christy Bressler or Aaron Johnson using the contact details provided under the left navigation menu.
FIELD DEFINITIONS
FIELD NAME REQUIRED? DESCRIPTION
Name of Provider Yes The full name of the church or organization
Address Yes The physical address of the church or organization
City Yes The city in which the church or organization is located.  Drop-down list is provided.  By default, Indiana is selected
Zip Yes The mailing zip code of the church or organization
Phone Number Yes The main contact phone number for the church or organization, not the individual contact
Fax Number No The main fax number for the church or organization
Website No The website address for the church or organization.  The web address should be entered with the full URL: http://www.website.com
Contact Person Title Yes The description of the position the main contact person holds with the church or organization. 
Contact First Name Yes The first name of the main contact person
Contact Last Name Yes The last name of the main contact person
Contact Phone Number Yes The direct line phone number for the main contact person
Contact Person Email Yes The personal email address of the main contact person
Re-enter Contact Email Yes Security feature to ensure the contact persons email address is entered correctly
Contact Person Details No Any pertinent information about the main contact person.  This may include location, job description, hours available for contact etc.
Side of Town Yes Geographically, which side of town the church or organization is located
Type of Church No see religious affiliation
Religious Affiliation No If the entry is for a church, this is the religion the church is associated with
Number of Members No A number indicating how many people are involved in the church or organization.  (Number may be an approximation)
Church Services No This field is provided so that churches can indicate when their services occur
Description No A descriptive account of the characteristics, features, purpose and/or subject matter relevant to the church or organization
* It is recommended to complete as many of the fields as possible on this screen.



HOW TO ADD PROGRAM INFORMATION TO THE DATABASE
To add programs that your church or organization provides into the database, click the link on the left hand side of the menu system titled "Program Information".  A series of textboxes and drop down menus are provided to allow you to enter detailed information about the programs your church or organization is currently offering or will be offering in the near future.  (You may also be automatically redirected to this page upon successful completion of entering your church or organizations details into the database).
 
If a red asterisk appears next to a field name on the screen, this denotes a required field. 



If you attempt to save the record and leave a required field blank, an error message will appear at the bottom of the screen telling you that this information must be completed in order to proceed.  

 

Once you have completed all the information and filled in all of the required fields on the screen, hit the submit button at the bottom of the screen. Upon successful submission of the information, the screen will be refreshed and you can begin entering the details for the next program your church or organization provides.  There is no limitation to the number of programs you may enter into the database.  The more information you can provide, the more comprehensive our resource guide can become to better serve the community!

To verify your program information has been saved correctly,  you may use the "Search Programs" page.  Choose to search by church or organization name, than locate your name in the drop down list.  After hitting the search button, a list of the programs you currently have entered into the database will appear where you can verify the information you have provided.

Church or Organization Yes A drop-down selection list of churches and organizations already in the database.  If your church or organization name does not appear in this list you must return to the "Churches & Organizations" page and submit the information before programs can be entered into the system.
Program Name Yes The full name of the program
Primary Contact Name Yes The full name of the primary contact persons name for the program being listed
Primary Contact Email Yes The personal email address of the main contact person
Re-enter Primary Email Yes Security feature to ensure the contact persons email address is entered correctly
Primary Contact Phone # Yes The direct line phone number of the primary contact person
Secondary Contact Name No The full name of the secondary/backup contact persons name for the program being listed
Secondary Contact Email No The personal email address of the secondary contact person
Re-enter Secondary Email No Security feature to ensure the secondary contact persons email address is entered correctly
Secondary Contact Phone # No The direct line phone number of the secondary contact person
Target Population Yes The particular group of individuals who are best suited to attend this program
Program Type 1 Yes The primary descriptor that best describes the type of program or service provided. If an appropriate program type is not identified in the drop-down list provided, specify other*
Program Type 2 No The secondary program descriptor (optional)
Program Type 3 No The tertiary program descriptor (optional)
Specify if Other * No If "Other*" is specified in any of the program type drop-down lists provided, a brief description of the program type should be included in this field
Program Location Yes The name and/or address where the program is taking place
Program Availability Yes The time and/or days, space available in the program
Program Description Yes A specific description of the program being offered
Program Accepts Volunteers? No Yes/No. Does this program accept volunteers to help plan/organize/support the program and its provider?
Continuous Program? No Yes/No. A continuous program would be one defined as not having a definitive end date
Program Start Date No If this program is not continuous,  the date the program is set to commence
Program End Date No If this program is not continuous,  the date the program is set to end or no longer be offered
